One of our nurse liaisons can arrange an evaluation to determine if your patient is appropriate for
transfer to the rehabilitation unit. They can also arrange for a tour or obtain
insurance authorization.
Nurse Liaison Service:
Nurses are available to evaluate people who have been referred for inpatient rehabilitation.
The evaluation is done on-site when possible, in the individual’s community hospital
or nursing home. Specific admission criteria is used to determine the appropriate
level of rehabilitation needed. Evaluations can also be done over the telephone.
