Sister Kenny Rehabilitation Institute offers a Falls Assessment/Prevention Clinic. This multidisciplinary program provides a comprehensive assessment of individuals who are at an increased risk for falls or fractures.
The goals of the program are:
to improve a patient's physical fitness and balance
provide interventions, including in-home analysis of fall risk factors
teach them how to reduce the risk of falling.
An osteoporosis risk assessment and bone density testing will be done, if indicated. We also help people identify appropriate assistive devices and footwear.
Assessment
A comprehensive assessment of individuals who are at an increased risk for falls or fractures will be done by a registered nurse. After an assessment, an individualized treatment plan will be developed, which may include referral to an occupational or physical therapist.
Family members are encouraged to participate in the assessment and subsequent treatment program. The referring physician will receive the results of the evaluation, including a proposed treatment plan and follow-up
recommendations.
